CSC Provincial Director Gina Crucio asked applicants to apply early because acceptance of application is on a first come, first served basis.

Crucio said the CSC Negros Oriental field office at Daro may close earlier than the January 13 deadline if the maximum number of rooms is already filled up.

The applicants are required to bring four pieces of passport-sized ID pictures with white background, showing a neutral facial expression and the ears; P500 application fee; and the original and photocopy of any of the following: valid IDs with photo, signature and birthdate, driver’s license, passport, Professional Regulations Commission ID, Social Security System ID, GSIS-UMID eCard, Postal ID, Barangay, Voter’s, PhilHealth or BIR (with signature of the issuing head), NBI clearance, company or school ID.

Any ID not in the list will not be accepted.

If the ID has no date of birth, CSC will ask for a copy of the NSO-issued authenticated birth certificate, but the applicant has to show the original one.
